Planning, paying for and improving health services are shared responsibilities. Health plans share them with families, providers, state agencies and community-based organizations. Based on that belief, the Toolkit is designed to focus the attention of health plans on children with special health care needs. Its 8-page booklet and 16 different tools can help plans improve their systems of care for this population. The tools are designed to be easily adapted for use by any health plan or provider group.
